We're there to advocate
Even when you can’t be there in person, we’re here to serve your loved one. While most tasks can be completed during regular business hours, we may be available at other times upon request. We can set up homemaking services, transportation, and attend medical appointments.
Initial assessment
We start with a free 30 minute interview to better understand your needs to schedule an assessment and create a care plan. This can be via telephone contact or a home visit. A comprehensive assessment can be scheduled to develop an individualized plan of care.
Navigate the provider system
Health care systems and services can be complex and confusing. They often require older adults meet rigid criteria for access. We can help navigate community resources and help to maximize existing insurance coverage to prevent them falling through the cracks.
Coordinate and monitor care
Jensen Case Management can act as a liaison between the client we serve, their family, and the providers to ensure individualized and customized needs are being met. Â You and your loved one, together with Jensen Case Management decide how often you need us.
Arrange for in-home services
In-home care is not always clinical or medical. We can help arrange reliable, dependable household chore services, transportation, shopping, bill paying, or home delivered meals.
