Your parent or loved one is no longer able to care for themselves. You struggle to keep up with regular visits, or live too far away to help with their daily care, or just check in with them daily or weekly. It's time to call Jensen Case Management LLC. We are the local professionals that can coordinate care for your loved one no matter where they live in Michigan's Upper Peninsula or Northeast Wisconsin.
We start with a free one-hour interview to better understand your needs to schedule an assessment and create a care plan. This can be via telephone contact or a home visit. A comprehensive assessment can be scheduled to develop an individualized plan of care.
Even when you can't be there in person, we're here to serve your loved one. While most tasks can be completed during regular business hours, we may be available at other times upon request. We can set up homemaking services, transportation, and attend medical appointments.
Health care systems and services can be complex and confusing. They often require older adults meet rigid criteria for access. We can help navigate community resources and help to maximize existing insurance coverage to prevent them falling through the cracks.
Jensen Case Management can act as a liaison between the client we serve, their family, and the providers to ensure individualized and customized needs are being met. You and your loved one, together with Jensen Case Management decide how often you need us.
In-home care is not always clinical or medical. We can help arrange reliable, dependable household chore services, transportation, shopping, bill paying, or home delivered meals.
Medicare, long term care, and other types of coverages for care needs vary greatly and can be challenging to understand, access, and navigate. Let us assist you and provide necessary and appropriate referrals.
